Home Based Support Services/Home Based Waiver Program provides a monthly budget for individuals and their families to get services through the Illinois Department of Human Services. We help individuals and families by assisting them in arranging for, directing and managing services. Funds often used to pay for a Personal Support Worker (PSW) who assists individuals with learning new skills. Funds may also be used for community day services, supported employment, home modification, adaptive equipment costs and therapies.

Care Center provides diapers and wipes to people in need

Lobby provides temporary shelter to the public from extreme cold weather.

Specialized health/medical services for veterans including primary and specialty care, surgery, mental health and substance abuse treatment, psychiatry, amputee care, audiology and speech therapy, dental and vision care, diagnostics, spinal cord injury treatment, and others.

Low Income Home Energy Assistance Program (LIHEAP) - electric and gas service payment assistance for low-income residents of York Township.

Determines eligibility to receive benefits under Social Security's disability programs, Supplemental Security Income (SSI).

Group living for teens that offer on-site counseling, therapeutic activities, educational support, vocational support, and life skills training.

Offers employment referrals, on-site evaluations, and case management services. Assists individuals in skill assessment, job training, career counseling, financial literacy, and workplace ethics training.
