Eligibility
SLF: Must be either a U.S. citizen or legal resident; Resident of Illinois; 22 years of age and older living with physical disabilities (as determined by Social Security Administration), or age 65 years and older; Screened by IHFS or an authorized screening agency and identified as someone in need of nursing facility level care; Cannot be participating in any other home and community-based services waiver; Income is greater than or equal to current maximum allowable amount of the Supplemental Security Income (SSI) and must contribute a fixed amount from that income monthly towards living, meal and service expenses. PACE: Live in PACE region, meet other requirements.
