People with urgent financial needs can get help through financial support and referrals. Case managers manage the Local Emergency Assistance Fund, along with funds from The Salvation Army and the Illinois Department of Human Services, to help those in need.
Case managers help patients and their families navigate treatment and payment, including understanding and applying for health insurance, appealing insurance denials, and connecting to resources for financial aid.
Assists with a wide variety of services and resources, including SNAP/Medicaid applications, TANF, ILHAF, applying for disability benefits, workers compensation applications, Court Based Rental Assistance (CBRAP) applications, child support applications.
Care coordination for children with eligible chronic health issues. Services coordinated include therapy, specialized equipment and transportation to medical appointments.
