End-of-life care and supportive services for patients and their families, including physician services, homemaker services, bereavement counseling, 24-hour emergency on-call services.

United States Passport Acceptance Facility. Applications accepted by appointment only. Applicants between 16-17 years of age need to appear with one parent. Both parents must be present with any child under 16 who is applying for passport.
