Application process
(1) Call the toll-free number during listed hours to speak to a Mission Coordinator. (2) Complete online application on website. (3) Get the doctor's release signed and returned. (4) Once a pilot has been found and confirmed by staff, passenger or designated person will communicate directly with the pilot.

Photo ID (driver's license, state ID, passport, or passport card); Proof of income; Physician's Release Form signed by treating doctor; Additional documents may be required.
Eligibility
Passengers must be able to walk on their own with minimal help; Must be able to get in out of a small airplane without heavy lifting or any special equipment; A doctor must verify that the person is medically stable and can safely fly in a small, non-pressurized plane; Must be experiencing medical or financial barrier to travel.
