Clubhouse Model Psychiatric Rehabilitation | Synapse House
Clubhouse Day Program to help those affected by brain injury and/or stroke regain work and daily life skills and build up self-esteem. Activities structured around 5-hours days include creating newsletters, computer skills, cooking skills, community outings to practice social skills, and exercise and therapeutic groups.

Application process
(1) Call office during listed hours to complete a phone intake and to schedule a tour, (2) Meet with the Admissions Coordinator to fill out an admissions packet, (3) A program orientation will be offered on the first day of the program, (4) Participants will be assigned tasks to complete at the agency.

Persons with a brain injury or stroke; Must be able to move with less than half the needed help; Must be able to must their own medications.
