Eligibility

Citizen of United States; full-time student; must be willing to agree to scholarship terms on provider's website. For undergraduate scholarship: must be under the age of 25, must have been diagnosed with and/or actively treated for cancer or chronic disease within the past 5 years. For graduate scholarship: must be pursuing a Master's degree or higher, must have been diagnosed with and/or treated for cancer or chronic disease within the past 10 years.

Required documents

Complete Scholarship for Survivors application (available on website) acceptance letter from college or university proof of income Diagnosis Verification Form completed by relevant medical professional official transcript letters of recommendation. Other documents may be required - please consult provider's website.
