Application process
(1) To apply in person, call the main number to find a location nearest you. (2) To apply by mail, download either the Senior or Persons with Disabilities application from the website and return the completed application to the address as listed on the application https://fares.rtachicago.org/. (3) If approved, a permit will be received by mail in about 3-4 weeks.

No Fees, Fees apply for replacement permits or for pictures to be take on site.
