Enrolled in the Illinois Department on Aging's Benefit Access Program; Persons aged 65 or older or with a disability; Meet income requirements.

Required documents

Government-issued Photo ID Previous year's tax return and/or income information color photo of yourself that is approximately 2 inches by 2 inches (Do not wear a hat, scarf, or sunglasses in the photo).

Application process

(1) To apply in person, call the main number to find a location nearest you. (2) To apply by mail, download either the Senior or Persons with Disabilities application from the website and return the completed application to the address as listed on the application https://fares.rtachicago.org/. (3) If approved, a permit will be received by mail in about 3-4 weeks.

No Fees, Fees apply for replacement permits or for pictures to be take on site.
