Residents of DuPage County 60 years of age and older; Must be homebound and have difficulty preparing own meals due to illness or disability.

Required documents

Proof of residence (driver's license, state ID, utility bill) Proof of age (driver's license, state ID, birth certificate, passport) Additional documents may be required.

Application process

(1) Call 630-407-6500 to confirm eligibility and sign up. (2) After the meal deliveries begin, a case manager from Community Services will make an appointment for a follow-up home visit.
