(1) Call to find information about your local Lions Club (2) Refer to Program Brochure for details on programs and services: https://lionsofillinoisfoundation.org/brochures/ (3) Obtain insurance medical clearance and audiogram (4) Download, fill out and submit application (5) Submit required documents (6) Give consent for service/authorize medical providers to release required healthcare information to Lions if approved.
