Eligibility

Reside in DuPage County; U.S. citizen or a Legal Permanent Resident, or DACA recipient; Does not qualify for Medicaid or Medicare; Able to enroll in a Marketplace Health Insurance Plan.

Required documents

Photo ID (passport, state ID, driver's license or matricula consular) Proof of income (such as paystubs, W-2 forms, Social Security awards letter or pension) Proof of address (utility bill issued within last 60 days, driver's license or state ID issued within last 30 days, current lease or proof of current mortgage).
