(1) Walk in during listed hours to request services, Or call, email or fill out online contact form and indicate that you would like to receive services or make a referral for someone who does (2) A staff person will confidentially collect and screen your referral information , including relevant documentation (3) The admissions team will work with you to schedule your free assessment, to be conducted in person or over the phone (4) Those who have been assessed and approved for admission will be scheduled for an intake (5) Refer to Admission Checklist on website to find out what to documentation and other items to bring
