Proof of your residence, such as utility bill with name on it
Proof of Social Security numbers for all people on your application
Proof of income for all members of family to be included in application, such as W-2 forms, tax returns, paystubs.

(1) Use the SNAP Eligibility Calculator on website to check if you and/or your family qualify (2) Apply for SNAP online or by downloading the application, completing it and either mailing it in or submitting it in person to local IDHS Family Community Resource Center.
