Provides a screening tool and online application for State of Illinois administered benefits programs. Programs include Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF), Aid to the Aged, Blind and Disabled (AABD), Medicare Savings Program, Medicaid, and Health Benefits for Immigrant Adults (HBIA).

Application process

(1) Use the SNAP Eligibility Calculator on website to check if you and/or your family qualify (2) Apply for SNAP online or by downloading the application, completing it and either mailing it in or submitting it in person to local IDHS Family Community Resource Center.

These may include, but not limited to: Valid photo ID, such as driver's license, state ID or passport; Proof of your residence, such as utility bill with name on it; Proof of Social Security numbers for all people on your application; Proof of income for all members of family to be included in application, such as W-2 forms, tax returns, paystubs.
