Eligibility
Resident of Illinois; Be pregnant or have a child under 19 years of age living with them; Child who is 18 years must be enrolled in high school full time; Be a U.S. citizen or meet certain immigration requirements.
Required documents
Photo ID (such as state ID, driver's license or passport)
Proof of address (such as a utility bill, copy of lease or mortgage statement)
Income and asset documentation (such as W-2 forms, tax returns, bank statements).
