(1) Call and complete an Animal Intake Request form over the phone, or (2) Complete Animal Intake Request form online at https://www.adoptpetshelter.org/clinic/owner-surrender/, save it and email it and a photo of the pet, as attachments to [email protected] (3) Intake Committee will review all owner surrender requests to determine suitability for adoption
