(1) Call 800-813-HOPE to speak with a social worker and complete an initial interview (2) Those who are approved as eligible based on the initial interview will be mailed/emailed an application and required documents will be requested (3) Application must be completed and submitted following the instructions on the website.

Proof of income: First 2 pages of signed income tax return (blacken out social security number for security); OR, Copy of most recent pay stub, unemployment check, SSI, SSD or public assistance benefit notification; OR, Letter of support from friend or family member in the absence of tax return or any other proof of income

Persons who have a diagnosis of cancer confirmed by an oncology health care provider; Being actively treated for cancer; Live in the United States or Puerto Rico; Income limits apply
