Persons who have a diagnosis of cancer confirmed by an oncology health care provider; Being actively treated for cancer; Live in the United States or Puerto Rico; Income limits apply
Required documents
Proof of income: First 2 pages of signed income tax return (blacken out social security number for security) OR, Copy of most recent pay stub, unemployment check, SSI, SSD or public assistance benefit notification OR, Letter of support from friend or family member in the absence of tax return or any other proof of income

Application process
(1) Call 800-813-HOPE to speak with a social worker and complete an initial interview (2) Those who are approved as eligible based on the initial interview will be mailed/emailed an application and required documents will be requested (3) Application must be completed and submitted following the instructions on the website.
