Eligibility

Guardians may be appointed, pending approval from the court, to serve as a substitute decision maker if a person is disabled because of: mental deterioration; physical incapacity; mental illness; or developmental disability.

(1) Call the intake number at 1-866-274-8023 and leave name and contact number to get a call back by next business day, or visit website to fill out online intake form at https://gac.illinois.gov/osg/online-intake-form.html (2) If calling from outside the State of Illinois, dial 1-708-338-7500 (3) If services are being requested, a Referral and Client Status form will be mailed out (4) Completed form returned by mail will be screened by an intake worker
