Application process

(1) Call 630-858-1005 Glen Ellen, and Wheaton 630-260-3773 to schedule an in-person assessment and to learn about the registration process, (2) Prospective clients/participants are encouraged to spend as much time as needed to observe existing clients and the various activities at the facility.

Medicaid/State of Illinois Community Care Program (CCP), Medicare; Self-Pay, VA health care, Long term insurance.
