Eligibility
Must be placed on the transplant waiting list at a Medicare-approved transplant center in Illinois; Must have received a transplant within the last 12 months before applying; Currently completing a transplant evaluation at a Medicare-approved transplant center in Illinois; Lived continuously in Illinois or Lake County, Indiana for at least 36 months before applying; Primary residence must remain in Illinois or Lake County, Indiana; Cannot be eligible for any other insurance subsidy or government insurance program; Household income is less than 138% of the Federal Poverty Threshold.
