Eligibility

Must be at least 6 years of age; Must reside in the following areas to qualify for sliding scale fees: Wayne or Winfield Township, including the towns of West Chicago, Winfield, Warrenville, Wayne, Bartlett, Hanover Park, and Carol Stream.

Required documents

Identification (driver's license, state ID, birth certificate) Proof of address (driver's license, state ID, current utility bill or rent agreement or mortgage statement with name on it) Proof of income for household (W-2 forms, tax return documents, paystubs) Additional documents may be requested during intake.
