Service Records (DD-214, DD-215, or ERB); Disability Decision (PEB, VA Rating Letter) if applicable; Photo ID (Driver's License, State ID, or Military ID); Proof of household income.
Eligibility
U.S. Veterans who served more than 180 days of active service after training; Immediate family member of a U.S. Veteran; Surviving family member of a U.S. Veteran.
